CSN STRIKE FUND REGISTRATION FORM

Please use the link provided below to register for the CSN Strike fund. The form only needs to be completed once and only takes a minute to fill out. It allows the CSN to print a check in your name if you participate in CUPEU's strike activities. Members who do not complete the form will not be able to receive the check.

Please note that this is not the CUPEU Roll Call form. All members are required to submit this new CSN form.

Members who do not participate in Week 1, but may in Week 2 are also invited to fill the form.
